Job Description Of Team Leader
Complete all service user assessment, including risk assessments as part of the assessment process, for new service users.
As required by the company's policies, routinely monitor, review, and update the care plan for the client.
To participate in a rotating system of being on call in case of emergency.
Should communicate often with care coordinators regarding hiring, distributing care workers, and developing care & support.
To keep a close line of communication open with the Care Coordinator and Registered Manager.
Support all field-based carer workers and ensure care workers in the field are observed, supervised, and mentored.
When a current care worker reports that they won't be able to make to attend to a call, assign a suitable replacement care worker.
To make sure that all written reports, administration, and data kept in a service user’ homes are accurate and current.
Make sure all client visits are always covered.
Conduct new care worker orientation visits with clients.
Immediately notify the care coordinators of any alterations to a care worker or service user's schedule.
