DescriptionSummaryUnder the direction of the Personal Care Coordinator, nursing and medical staff provides direct care services for residents/clients while keeping them safe and comfortable by performing the following duties.Essential Duties and ResponsibilitiesTeamwork with the following and all other duties and responsibilities assigned.Effective leadership that includes supporting core values, addressing zero tolerance behaviors and maintaining active communication with employees supervised.Provides primary care including all ADL’s, including but not limited to bathing, dressing, toileting, restorative care, and vital signs.Performs random rounds on each resident upon arrival for shift and prior to departure.Coordinates daily Care Aide assignments and scheduling considering patient acuity and overall censusAccurately document care provided to residents using approved documentation method.Acts as an intermediary between RN/LPNs, personal care staff, and residents/family membersMonitor Care Aide work performance for efficiency, effectiveness, and attendance,Provide coaching and counseling at the request and under the guidance of the Personal Care CoordinatorRegularly take a resident assignment on the unit to ensure adequate staffing for proper direct care deliveryTakes and records temperature, pulse, and respiration rates, and food and fluid intake and output, as directed.Other ResponsibilitiesAssists with stocking supplies in clean utility rooms..Attends meetings and training sessions required for the positionOther duties as requiredRequirementsSupervisory ResponsibilitiesCNAs/Care AidesQualificationsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. May need additional skills with special-need geriatric residents such as those with dementia, Alzheimer's or those requiring skilled health care.
