Main duties of the jobMoor Croft Ward is an acute assessment ward for people aged 65 years and older who are experiencing functional mental illness, many of which are supported home on their discharge from the ward. Main duties include:
Maintaining patients dignity, privacy and safety at all times. Assessing risk Adhering to mental capacity and mental health act.
Recognising potential signs of harm and ensuring patients are safeguarded.
Maintaining high professional standards.
Delivering and planning evidenced based care in collaboration with the patient, families and wider MDT. Assessing patient need and delivering appropriate and timely interventions.
Safe administration and storage of medicines.
The MDT team consists of medical staff, psychology, occupational therapy, physiotherapy, speech and language services, dietetics and physical health care, due to this we are able to offer a comprehensive assessment of each patient and their individual needs. Daily MDT report outs and discussions aid the holistic approach to our care on Moor Croft Ward and contribute to the effective treatment and management of each patient, whilst considering discharge planning throughout their in-patient journey.About usWorking for TEWV NHS Trust you can expect fantastic training & development opportunities with the strong links we have developed at our local universities'. It involves the delivery of high quality interventions to meet the needs of the patient, developed in collaboration with the patient, their families and the wider MDT whilst following best practice guidelines. This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Evidence based knowledge of best practice in caring for patients in the designated field. Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act) Understanding of Safeguarding and its application in practice Clinical Risk Assessment and Management and its application in practice Clinical Governance and its application in practice.
