CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting! Are you an experienced Physiotherapist looking to utilise your clinical experience within a community setting? Do you want to develop your management and leadership skills within a fast-paced operational environment? This job opportunity will see you work closely with multi-organisation partners to improve the patient experience, timeliness whilst facilitating appropriate discharge destinations. If you can answer yes to any of the above then we may just have the job for you!Main duties of the jobOur DTA Beds team has secured extra funding and we are looking to expand. The DTA Beds Therapy Team provides ongoing therapy and facilitates discharges. This includes managing caseloads across multiple care homes in South and West Hertfordshire. We also complete home visits to organise equipment for discharge home all in a timely manner with the assistance of social services. We are looking to recruit an experienced Phsyiotherapist to co-lead the Discharge to Assess (DTA) bed therapy service within West Hertfordshire. If you have experience of leading within health services and have a desire to further develop a service in a highly performing, stimulating and fast-paced environment then this may be your opportunity. Current AHP with UK HCPC registration Diploma/degree in Physiotherapy Evidence of up to date CPD portfolio Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation or older people

Post graduate degree in relevant area Member of a Special Interest Group relevant to this post Member of BAOT
ExperienceEssential
Experience working at a senior level in a wide range of clinical areas including, neurology, general rehabilitation, falls, elderly medicine.

Experience of wide range of specialist services eg. falls services, wheelchair services, and learning disabilities Experience working as a Band 6 Physiotherapist Experience of community rehabilitation Experience of working in the NHS
Skills & KnowledgeEssential
Skills in the assessment and treatment of adults with complex therapy needs Knowledge and understanding of complex medical conditions including ie. neurology, multi-pathologies and falls Knowledge of role of Allied Health Professionals and interdisciplinary team working Ability to apply core physiotherapy skills across a range of clinical conditions within a client centered, goal setting Knowledge of outcome measures and evidence based practice relevant to Physiotherapy practice in community Knowledge of the NICE falls guidelines, National Stroke Strategy, Clinical Guidelines for Stroke and the National Service Framework for Long-term Conditions Ability to problem solve, prioritise and demonstrate clinical reasoning Good communication and report writing skills, in both verbal and written format Teaching of junior staff / students, peers in in service training setting Knowledge and understanding of the role of Social Services and voluntary agencies

Supervision of assistant / junior staff Knowledge and experience in research and audit Knowledge and understanding of National Service Framework for Older People, NICE Guidance and Clinical Guidelines for progressive conditions Knowledge and understanding of NICE and CSP/COT Fallers Guidelines
