This is an exciting opportunityto collaborate with health colleagues in a multi-agency integrated service focused on Health and Social Care. The aim of this role is to reduce unnecessarysocialhospital admissions/ to supportcarer breakdown and to promote independence, by assessing people in their own homes and assisting people to maximise independent living.
Our focus includes community support before hospital transfer andassistance in Accident and Emergency departments.As a Level 2 Social Worker, you willassistthe manager indelivering a holistic service, ensuring the safety of vulnerable adults, andfacilitating promptassistance for adults and their caregivers during crises such as falls or caregiver emergencies. Your responsibilities will involve assessing needs for eligibility, devising care plans, and achieving ormaintaining well-being outcomes.
We areseeking a motivated, organised Social Worker with energy and creativity.You will need to beregistered with Social Work England, along with strong interpersonal and communication skills.You will have an up to date knowledge of relevant legislation, and an in-depth understanding of relevant adult social care legislation.
