Responsibilities:
Conduct child and family assessments collaboratively with the child, parents or carers, and other agencies, ensuring the child's voice is central to the process and risks are appropriately analysed.
Develop outcome-focused Care plans and/or Pathway Plans in consultation with children, young people, and/or their parents or carers, prioritising the child's needs, safety, and stability.
