Social Worker - Safeguarding Continuing Healthcare

Requirements:

• Social Work England Registration
• Social Work Qualification
• Knowledge of the Care Act and experience of undertaking safeguarding enquiries, investigations and risk assessments
• Extensive experience working vulnerable adults

This local authority based in London is seeking an experienced Social Worker specialising in Safeguarding CHC and complex case management who can join the front door team.

They are currently working in a hybrid way with 1-2 days based in the office and the rest WFH. Staff are however expected to undertake home visits.

Delivering a high-quality social work service to older adults with both physical and complex mental health problems, you will be responsible for their care coordination and case management. With a focus on improving their wellbeing, you will promote their recovery and independence wherever possible, balancing this against the need to protect their safety.

You will carry out Care Act strength-based assessments and reviews to support people in choosing outcomes which are both preventative and well-suited to their current needs, and you will ensure they are supported in managing their identified risks. Safeguarding this group of vulnerable service users is key to this post.
