Opus People Solutions on behalf of Cambridge County Council have an exciting opportunity for a Social Worker to join thier North Transfer of Care Team,
You will be based at Hinchingbrooke Hospital with cover needed sometimes to assess people at Peterborough City Hospital and Queen Elizabeth Hospital Kings Lynn. Hours are Monday to Friday 9-5pm and 4.30pm on Fridays.
There is the opportunity to work from home a couple of days a week once the worker knows the role, however the worker still may be required to come into the hospital on those days to assess, if demand requires it.
In this role you will be:
Assessing people in Hospital for care packages or care home placements under the care act.
Assessing the Mental Capacity for care and support, care planning, carers conversations, completing making safeguarding personal discussions, working alongside the discharge planning team and other services to ensure quick and safe discharges, taking part in the duty and duty back up rota as required.
