Social Worker - Community/Hospital Discharge Team

Outstanding rate | Low Caseload | Flexible Working
You’ll have the opportunity to work flexibly in a supportive team of managers and social workers. This means flexi-hours are an option and even compressed hours – this will give you the opportunity to manage your own diary around any personal/childcare commitments you may have.
An outstanding rate of up to £38 per hour can be offered for this post, as Leaders also offer an exclusive loyalty bonus as a thank you for all of your hard work.
You will also be given the chance to work from home for up to 2 days per week, giving you a better work-life balance and that much needed time to do any write ups you may have – although you will only be holding a small caseload of up to 20 care users.
You will be working collaboratively with other healthcare professionals to develop and implement care plans, coordinating services and resources, and advocation. In addition, you may conduct assessments to identify social and psychological needs, provide education on health-related issues, and work to promote health equity and social justice.
