Social Worker - CHC Team

Opus people solutions on behalf of Cambridge County Council have and exciting opportunity for a qualified Social Worker to join their CHC team.
This role will be Hybrid working and will require you to do two days per week in the office, which is based at Scott House, 5 George Street in Huntingdon.
You will also be doing Visits in the community and care home settings.
This role will include:

• Implement the completing Care Act 2014 Assessments, Reviews, Mental Capacity Act 2005 Assessments and Best interest Decisions (where applicable) identifying individuals needs and supporting the Multidisciplinary teams' recommendation on eligibility for NHS Continuing Healthcare. Where individuals are not eligible for NHS Continuing Healthcare but are eligible for care and support from Cambridgeshire County Council in line with the Care Act 2014.

• Work in partnership with Cambridgeshire and Peterborough Integrated Care Board (ICB) and determine individual's eligibility for NHS Continuing Healthcare through joint completion of the NHS Continuing Healthcare process.
  
• Provide more specialist social work practice skills and knowledge to make independent decisions and support situations in which risk, ambiguity or complexity is greatest.

• Take a professional lead and oversee cases of individuals who lack capacity to agree to or plan their care, undertake proportionate assessment in accordance with current Adult Social Care legislation including Mental Capacity Act and Best Interest assessments.

• Support in safeguarding process to assess and manage risk, knowing how to intervene proportionately and ensuring people are protecting from harm, while protecting their human rights.
