This Senior Practitioner opportunity would be suitable for a Band 6 practitioner looking to develop their leadership skills & experience or a Band 7 looking to broaden their range of experience. It is open to Nursing, Social Work or Occupational Therapy staff. City & Hackney Rehabilitation Team is a specialist community mental health team that provides clinical care for service users with severe and enduring mental health needs living in Hackney. In addition, we provide regular care act assessments/117 reviews for mental health service users placed in supported accommodation. The teams focus is on supporting service users towards greater independence, building their confidence and ultimately enabling service users to step down to less supported or more appropriate accommodation.Main duties of the jobThe Senior Practitioner will provide leadership, advice and support to the team ensuring that the service provided is high quality and person centred. They will also be expected to deputise for the Operational Lead in their absence. The Senior Practitioner will be expected to have or to develop expertise in accommodation & support available to service users living in the community. The post holder, as well as undertaking placement reviews and holding a small caseload of complex cases will be expected to provide advice and liaison regrading community support options to colleagues in community and in-patient services. They will be expected maintain and develop relationships with key stakeholders including senior managers and staff from both the East London NHS Foundation Trust and London Borough of Hackney, commissioning, LBH legal, the ICB, homelessness and housing advice services, Look Ahead and other housing providers. Substantial community experience of providing mental health care at Band 6 or above Experience of working in a multi-professional team Significant experience of leading clinical work including experience in managing the review of placements Experience of working with housing organisations who deliver supported housing to people with a history of mental illness Experience of developing services and the change process

Experience of supervising and appraising staff Experience of deputising and leading an MDT in the absence of the team manager
