Senior Nurse

The Urgent Community Response Service provides nursing, therapy and reablement to adults to support them to remain in their home avoiding attendance and admission to hospital and to return home following a hospital stay.Operating 365 days per year, the service receives referrals from a range of health and care professionals and triages, assesses, and delivers care to the patient over several days to ensure the patient can remain safely at home.The post-holder will be required to provide clinical expertise and problem solving support for the nursing team as well as clinical care for patients requiring nursing care. They will co-ordinate the multi-disciplinary team, allocating visits as appropriate. There is an expectation to develop the service through reflective practice, clinical supervision, and audit. The postholder will identify suitable people referred to the service for planned early discharge from hospital or for the prevention of admission to the acute sector or long-term care.Main duties of the jobTo assess, plan and provide clinical care needed for patients to be admitted to Urgent Community Response.To work as part of a multi-disciplinary / multi-organisational team and liaise with consultants, hospital and social care staff and other agencies as appropriate.The post holder will be expected to develop liaison skills to meet the requirements of the service.Co-ordination of the team, allocating visits on a daily basisAttendance at handover and caseload meetings.Provide clinical expertise and problem-solving support for the nursing teamAbout usWe are the main provider of NHS community services across East and West Sussex.
