The role of Senior Nurse practitioner to work in partnership with service users and their carers in the Older People Community Mental Health Service with complex health presentations including dementia, to lead the team, to manage the day to day running of the service, to ensure that patients have personalised care plans to enable them to lead fulfilling lives. The Senior Nurse practitioner will demonstrate an understanding of clinical interventions relevant to Older People and people with dementia and their carers to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this.The post holder must be a NMC registered Nurse (Mental Health) with experience of managing teams at band 7 level or seeking an opportunity and believe they have the required skills and ability for the post.Main duties of the jobTo Support the Team Manager and take clinical leadership of the team.Complete case Allocations for Initial assessments and caseload review for the CPN's as part of caseload management.Provide Leadership on the MDT team Meeting in conjunction with the Team manager and Consultants.Participate in delivering in-house training with the team by working in partnership with the Leads.Support with HR processes and the Health Roster system for the management of staff well-being and monthly payroll system.To provide some comprehensive assessment of mental health specific to Older People with mental ill health including dementia.To manage a small caseload of clients with complex mental health needs/dementia and undertake case management/lead professional/ CPA as needed.To lead on a shared duty service.To be able to communicate with and meet the needs of families and carers as appropriate to the care group and the individual client.To have experience of and specialist knowledge on interventions to be able to lead in clinical area.To have a knowledge of both organic and functional illnesses.To receive management supervision from team manager or equivalent.To provide supervision to junior staff on a monthly basis.To demonstrate excellent communication skills.About usWest London NHS Trust is one of the most diverse healthcare providers in the UK, delivering a range of mental health and physical healthcare and community services.
