This is an excellent opportunity to develop your clinical skills and knowledge alongside service development, within the Core20plus5 framework of long-term conditions.Main duties of the jobThe post holder will work as a complex case manager, with direct supervision. You will be involved in delivering integration, working with GP's, Social services and the third sector organisations. You will be providing case management, clinical staff support, mentoring, clinical assessments for people who have difficulty accessing healthcare services and with some of the social determinants of health inequalities for the most vulnerable communities, supporting the service at GP practices, community outreach events and where appropriate, supporting healthcare closer to home. The post holder will be a Registered Mental Health Practitioner, with at least 2 years community experience of managing a complex caseload of patients. You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing assessment skills, to manage their conditions in the community. He/She will work as part of the health inequalities clinical team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients.

Registered Mental Health Nurse (RMN) with current UK NMC registration Degree/Diploma in Adult Nursing Evidence of leadership courses/training Training on effective appraisal

Knowledge and ExperienceEssential
Experience of working and delivering healthcare to patients with complex needs or hard to serve service users in the community. Evidence of theoretical knowledge and practical experience in a wide variety of clinical areas within secondary and primary care Experience assessing, planning and implementing care plans Experience of providing evidence-based care in the community Recent evidence of liaising and negotiating with system partners to facilitate quality outcomes for patients Experience of organising and managing team caseload Experience of teaching and mentoring junior staff and undertaking appraisals Knowledge and experience working within Health Inequalities. Experience managing long term conditions in the community Partnership working with multi agency/system partners to deliver proactive effective seamless services in the community Evidence of Research Based Practice and service innovation Flexible approach to change and service innovation. Recent examples of service innovation and change management experience. Experience of using a range of Clinical / IT systems Experience of working with MDT and System partners across organisational boundaries Working knowledge of National policies and procedures across health and social care and its implementation or application
