An exciting opportunity has arisen at Six Physio, where we are looking for an experienced Senior Domiciliary Physiotherapist to join our team: caring for patients in their community and within residential care homes, across Central, South-West and North-West London.

ROLE AND RESPONSIBILITIES
Reviewing patients' medical history and relevant operation notes
Developing a personalised treatment plan for each patient, outlining their goals and expected outcomes. Consulting with colleagues, family members and other members of the MDT
Planning, implementing and managing patients' rehab plans
Recording and evaluating patients 'progress on TM3 system
Complying with HCPC and CSP regulations, standards and code of conduct
Providing reports to Insurance companies, colleagues as required
REQUIRED SKILLS
Exceptional communication and interpersonal skills
Excellent time management
In depth knowledge of neurological and orthopaedic conditions and treatments, and aged care/rehabilitation
PREFERRED SKILLS
Neurological skills and experience
Orthopaedic skills and knowledge
Hydrotherapy experience desirable
Experience of working with the elderly and patients with dementia
QUALIFICATIONS AND EDUCATION REQUIREMENTS
BSc or Equivalent in Physiotherapy
Up to date record of CPD
ADDITIONAL NOTESThe successful applicant must have experience of lone working and managing travel between patients.
An to date DBS certificate is preferred.
