We are currently looking for registered nurses with provenexperience of working within a district nursing service for the role of seniorcommunity staff nurse. This includespalliative care, wound care, catheter care. If you feel you have the skills andexperience to be able to manage a cohort of higher acuity/complex patient asdirected by the team leader (caseload holder) we would welcome yourapplication. Applicants will need to demonstrate excellent communicationskills, and a sound ability to prioritise, time manage their own patient visitlist, complete complex holistic assessments, lead on developing plans of careand supporting/mentoring junior staff members within in the service. Allflexible working requests will be considered.Main duties of the jobThe post holder willwork as a senior member of the District Nursing Team collaborating with theTeam Leader (caseload holder) and support the provision of clinical leadershipwithin the team. A key feature of the role is to support the proactive managementof patients with complex or higher acuity conditions on the District NursingCaseload. This will include collaborating with other members of themulti-disciplinary team involved in the patients care to establish aperson-centred approach to management of health/social care needs, that alsopromotes self-management. They will require the use of extended clinical skillsand expert knowledge to assess the physical and psycho social needs of adefined patient group, instigating therapeutic treatments based on bestavailable evidence in order to improve health outcomes. In doing so, they willassess, plan, deliver and evaluate patient centred programmes of care ensuringthat they practice within their sphere of competence and knowledge. The postholder will work to professional and NMC codes, standards and guidance at alltimes ensuring that their practice is based on the most up to dateevidence-based theory and practical knowledge.
