Job overview
An exciting opportunity has arisen for the recruitment of a Senior Community Nurse within an integrated community team based at Far North Neighbourhood, Thornton Cleveleys. The advertised role is a permanent position. This appealing role will enable the successful person to support the integrated leadership and management of staff and the caseload within the community to meet the needs of the population.
Interested applicants will need to be registered nurses who have relevant nursing qualifications to degree level, specialist post registration education and leadership training or equivalent experience. A minimum of two years post registration is required.
Main duties of the job
We are looking for the successful candidate to be able to:
assess, plan, implement and evaluate care for a defined district nursing caseload
ensure that the care needs of the caseload are met by effective management and appropriate delegation of work to other members of the team
work in partnership with the primary health care team, liaise and cooperate with everyone involved in holistic patient care
be able to demonstrate extensive knowledge, advanced analytical and practical skills, and the ability to communicate in a non-judgemental, empathetic way with patients, carers and other professionals
Provide clinical supervision to junior staff and support staff development
The post holder will need to have good communication skills both written and verbal, have evidence of ongoing professional development and be able to work independently.

Detailed job description and main responsibilities
To assess, plan and evaluate the individual requirements of patients and carers referred to the District Nursing team. Diagnose specific conditions including venous insufficiency, skin conditions, fungal infections, constipation; recognise uncontrolled symptoms - especially in palliative care, and implement treatment regimes using prescribing when appropriate.
To provide and document a high standard of evidence based nursing care to patients in the Community.
To delegate work to members of the Nursing Team, taking into account Scope of Professional Practice, and nursing skill mix. To manage the caseload, triage visits and complete senior reviews within the community setting.
To work within NMC. Guidelines relating to Code of Conduct, Confidentiality, Accountability and Scope of Professional Practice.
To facilitate effective communication, written, verbal and electronic, whilst acting as the patients advocate. Be able to communicate empathetically, sensitive potentially controversial, and/or distressing information to patients and their carers, e.g. relating to patients entering the end stage of life
