The Band 7 Senior Assessment Practitioner will be based in the Lambeth Home Treatment Team and will ensure provision of a rapid assessment function across crisis, acute and community services to optimise patient flow and prevent delays in patients getting the right care at the right time. This will involve working closely with the Acute Referrals Centre, Liaison at GSTT, Lambeth in-patient wards and our community teams to make sure patients in need are identified and assessments are carried out promptly and effectively. The Senior Assessment Practitioner will oversee all referrals and assessments into HTT, provide outreach to community teams to help identify patients in need of crisis care and in-reach to the wards in collaboration with the borough Patient Flow Coordinator to identify any barriers to patients being discharged in a timely fashion. They will be flexible and provide support when needed in the assessment and management of care for those Lambeth Patients placed with private in-patient healthcare providers. They will also help monitor the impact of the role on patient flow across the borough and will contribute to QI and service improvement initiatives to ensure rapid and effective assessment for all patients as needed.Main duties of the jobTo provide a rapid assessment function across acute, crisis and community services and prevent delays in patients getting the right care at the right time To take a lead in the Home Treatment Team for managing all referrals and assessments To work across borough crisis and community services to ensure patients are seen promptly and the right care pathway is identified. This includes working closely with the Acute Referrals Centre (ARC), the Liaison team at GSTT, Lambeth in-patient wards, the borough Patient Flow Coordinators and community teams. To work as part of a management team to oversee all referrals for assessment into HTT. To work with stakeholders to identify and help resolve barriers to discharge. To provide link working with borough community teams to identify patients for HTT at the earliest opportunity. To be flexible and responsive to pressures across the pathway, carrying out assessments as needed. This will include those patients in private overspill beds as needed To be flexible within the team to help ensure the smooth running of HTT from shift to shift. This may include being available to support the team in carrying out care for existing patients where there are shortages or capacity concerns To be in daily contact with the ARC. This will include helping to ensure an accurate record of all referrals and referral outcomes, making sure assessments are allocated and carried out in a timely fashion and having an up-to-date awareness of the current caseload and capacity of the Home Treatment Team To work closely with the flow coordinator and Lambeth wards to identify patients and help resolve barriers to discharge. To provide direct personalised link working with borough community teams to identify patients for HTT at the earliest opportunity and make sure assessments get carried out promptly. To be flexible and responsive to pressures across the pathway, carrying out assessments as needed. This will include those patients in private overspill beds as needed, in liaison with the teams private overspill practitioner To be flexible within the team to help ensure the smooth running of HTT from shift to shift. This may include being available to support the team in carrying out care for existing patients where there are shortages or capacity concerns To be in daily contact with the ARC to ensure effective communication and prompt assessment. Recognised Professional Qualification in Nursing (RMN), Social Work (Dip SW/CQSW) or Occupational Therapy Post registration qualification in specialty Recognised leadership qualification or equivalent experience
