The main purpose of the role is to provide support within the Trusts Units to ensure the statutory duties and functions associated with patient safety and effective clinical governance are fulfilled. The post holder would be part of the Corporate Patient Safety with links to the Business Units, in that they would also be an integral member of the senior team to ensure the development, co-ordination, implementation and evaluation of patient safety and governance functions.Main duties of the job
The post will work collaboratively with staff across all levels and disciplines within the Trust, and with key stakeholders, e.g. ICB colleagues. The post holder will routinely work with senior managers and senior clinicians, supporting them with delivering improvements in their areas. The post will be responsible to the Head of Risk and Patient Safety. The post holder will work within the Patient Safety team to ensure a consistent organisation-wide approach to incident reporting and management. This will include producing written reports to a high standard and delivering presentations to deliver key messages regarding the Quality agenda. Provide information and analysis on key themes regarding clinical governance i.e., patient safety events, learning responses and a range of data that impact safe and effective patient care, ensuring this is shared across the business unit and wider organisation.

Provide expert advice, guidance and support to all staff on matters related to patient safety incident investigations and responses, ensuring compliance at all times with local and national guidance. Work autonomously as a patient safety specialist/ learning lead and in conjunction with subject matter experts and clinical colleagues, escalating any identified risks to the Head of Risk and Patient Safety. Encourage and support the Trusts statutory duties in relation to Duty of Candour , working closely with the Legal team and providing education and support to clinical colleagues in these conversations. In conjunction with subject matter experts, identify and review all sources of information in relation to patient safety incidents and use a variety of analytic skills to identify systems learning. The information may include paper and electronic records, witness statements, liaising with stakeholders, patients, their families and carers. Apply the principles and practice of embedding SEIPS methodolgy as part of patient safety learning responses. To represent the Business Unit at key corporate meetings i.e. Risk and Patient Safety Council and Safecare Council (or equivalent), ensuring effective dissemination back to the Business Unit from these forums, and vice versa. To support the Business Unit by leading the strategic direction of patient safety, providing advice and oversight to ensure that all patient safety events and near misses are reported and managed in line with Trust policy and national guidance, and that all potential learning opportunities are maximised. Support the Patient Safety and Business Unit teams in the development of SMART action plans to ensure learning is embedded and sustained into clinical practice working collaboratively with clinical partners. To be a member of Trust Patient Safety Learning Panels. To be aware of risks on the risk register for allocated areas and where needed liaise with the Corporate and Clinical Risk Lead to ensure mitigations for these are appropriate. Encourage a just culture of openness, transparency and learning that responds rapidly to potential or actual harmful events within health services. To ensure that recommendations and actions from Learning response are integrated into the Business Unit quality improvement plan to ensure consistent and demonstrable improvements in the quality and safety of patient care. To work closely with the Quality team, triumvirates and Safecare Leads within the Business Units to facilitate a coordinated approach to the delivery of the Business Unit quality improvement plan To undertake and deliver a programme of education and training which supports the NHS Patient Safety Strategy. Support the development, implementation and monitoring of national and local safety initiatives.

Sound clinical knowledge Significant knowledge and experience in patient safety and incident management Significant knowledge and experience of the patient safety investigation process Commitment to CPD Sound knowledge of Human Factors Ability to manage conflicting opinions from a number of different internal and external sources when managing investigations, and using diplomacy and skill to reach a conclusion Ability to present information in a clear and logical manner and in report form to a very high standard Ability to provide and receive complex, sensitive and contentious information

Qualification and TrainingEssential
Significant experience of working in NHS and within the field of clinical governance At least 2 years' experience in a senior role (Band 7 and above) Evidence of leadership development Educated to Master's level or equivalent relevant to practice or willingness to work towards Evidence of ongoing professional development at post graduate level Ability to facilitate the learning of others.
