Reablement Social Worker - Flexible Working

Requirements:

• Social Work England Registration
• Extensive Adults Social Work Experience
• Experience completing assessments of needs, long term care plans, MCA & Best Interest, Risk Assessments, Safeguarding

This local authority based in the North West of London is seeking an experienced candidate to join the Reablement Team. The team work with customers, who have potential to regain their independence. we work closely with health teams. We also complete assessments of needs, long term care plans, MCA & Best Interest, Risk Assessments, Safeguarding.

Social Workers are expected to work in the borough 2 days per week at present, and the role involves frequent home visits.
