Qualified Social Worker | Discharge to Assess (D2A) Team | Lewisham Qualified Social Worker required to join a statutory Discharge to Assess (D2A) Team Hybrid working 9-5 Monday to FridayOffice base: University Hospital Lewisham, Lewisham High Street, London SE13 6LH. The nearest train stations are Ladywell (3 minutes walk) and Lewisham Station (15 minutes walk).You must have a social work qualification and full SWE registration as well as relevant experience to be consideredPay rate £30 - £32phThe Discharge to Assess (D2A) pathway is an integrated multi-Disciplinary service which provides support and rehabilitation to adult Lewisham residents to return them to an optimum level of function after an admission to hospital. The team is made up of staff from health and social care (social workers, therapists, and nurses), working together to provide a holistic assessment and support plan to ensure outcomes are achieved and independence is maintained wherever possible.In this role as a Social Worker, you will be ensuring that citizens with care and support needs receive community care and support in accordance with the Care Act 2014 always ensuring and strength-based approach and person-centred support plans. Additionally, you will be identifying and carrying out Mental Capacity Assessments and chair best interest meetings as well as protect adults at risk of abuse in accordance with Lewisham Council Safeguarding Policy, the Care Act 2014 and making safeguarding personal. There is a duty rota in place which will allow you to both working from home and being based in our main office (Nursery Building - University Hospital Lewisham).
