The post holder will represent Brent at North West London discussions in order to implement sector wide transformation work streams locally.The postholder will lead the development and implementation of the Community health services in Brent by working collaboratively with the Community Service Providers, Primary Care Networks, acute Trusts, Adult and Social Care team and the Voluntary Sector.The post will liaise with the North West London Integrated Care Board (ICB) Team to ensure local implementation of sector wide work streams.About usBrent Integrated Care Partnership(Brent ICP) brings together health and care organisations from across the borough.
