Patient Pathway Coordinator Breast ServicesBand 4Job overviewThe post holder will be responsible for supporting Breast Services patients through their pathways, with an emphasis on tracking patients through their diagnostic investigations, as well as tracking patients under surveillance who require planned investigations, to ensure faster diagnosis and efficient pathways. This post will focus on patients on the Breast Services pathways and models of care.Main duties of the jobMain Duties/Key Results Areas: To schedule theatre lists in liaison with patients, consultants and capacity. To ensure all appropriate pre-operative assessments have been carried out to allow theatre lists to run as planned.To manage/validate large waiting list and to assist in ensuring all patients are operated on within the maximum waiting time target.Ensure that patients are informed with reasonable notice, and that all relevant paperwork is completed for given admission dates. This included ensuring that all elective attendance/admissions are promptly and accurately recorded on the appropriate Iclip systems.To take responsibility for reviewing and validating the wait list on a regular basis and to ensure that appropriate follow up action is taken.To validate a weekly patient tracker in managing the service targets for total booked DSU and inpatient.Coordinate surveillance and monitoring through appointments and investigations and feedback results for non-complex casesMaintain accurate written patient records, which reflect the care givenInform the team/person in charge of: -any changes in the patient’s physical condition or situationany significant factors relevant to the patient.Working for our organisationJob Title: Breast Services Pathway CoordinatorBand: 4Hours of Work (p/w): 37.5Service Centre/Directorate: Diagnostics, Breast Service, Genetics and Mortuary DirectorateBase: St George’s Hospital but the post holder may be required to work at any of the Trust’s sites.Key working relationships: Breast Surgeons, ANP’s & PA,s, Deputy General Manager Service Manager, Assistant Service Manager, Clinical Team, MDT Coordinator, Cancer Team, Breast Services Care Group, Outpatients.Detailed job description and main responsibilitiesThe Patient Coordinator is responsible for the operational delivery of their Specialities inpatient admission service. The post holder is responsible for the patient overseeing patients through their Breast Services pathways. The post holder will be responsible for coordinating the Breast Services TWR and Breast Imaging clinics ensuring all patients are booked in line with the Cancer RTT national target. The post holder will be responsible for tracking all patients on the Breast Services pathways, attending weekly meetings and ensuring all diagnostic investigations are completed within the Breast Services pathways’ deadlines to The post holder must be able to work on own initiative and be prepared to work at both SGH and QMH. The post holder will be expected to work closely with the Service Lead to help with the coordination of imaging and surgical capacity in the Breast one stop clinics.
