The main focus, purpose and impact of the work are individuals (patients and clients) and groups (including carers) or communities meeting their health, health related and nursing needs.
(These will be defined and agreed by the service).
Deliver an expert nursing service within the specialist area, and mange a caseload of patients. Assessing patients, planning and implementing care, providing specialist advice maintaining associated records be a specialist in a defined area of nursing care provide specialist education and training to other staff and students. Use knowledge and skills as a basis from which further to develop practice, understanding the implications of the social, economic and political context of health care, including the assessment and management of risk. This will be achieved by engaging in flexible, cross-boundary partnership working facilitating the care of this group of people in the community. Contribute to the development of evidence based clinical nursing practice in line with current research and guidelines.
Particular emphasis is put at Kings on the need to ensure that all staff recognize their responsibility to deliver services in a high quality, courteous, patient focused manner maintaining patient confidentiality at all times Main duties of the job Provide specialist nursing service for children and young people with haematology specialist needs providing expert nursing advice and support to patients, their families and other healthcare professionals following diagnosis and through treatment. Ensure all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made. Assess health, health related and nursing needs of patients or clients, their families and other carers by identifying and initiating appropriate steps for effective care for individuals and groups. This can include:
Making and receiving referrals
Ordering diagnostic tests Managing a patient caseload
Liaising over admitting and discharging patients for specified conditions and within agreed protocols
Prescribing medicines and treatments Running nurse led clinics Paediatric Apheresis service Conduction telephone clinics
Attend/run outpatient clinics to follow up patients and ensure all relevant information is available to enable patients to make an informed choice about their treatment. Assess and manage critical and clinical events to ensure safe and effective care. Support and empower patients and clients, their families and other carers to influence and participate in decisions concerning their care by providing information on a range of specialist nursing care and services. Ensure all care is given in accordance with agreed protocol including working within a suitable mechanism for MDT referrals to the Clinical Nurse Specialists.
2. Provide support and guidance to others so that they are able to deliver a customer focused service
3. Coordinate and attend MDT meetings
4. Ensure involvement in cross-department processes and planning
5. Regularly review those clinical notes of patients requiring imminent admission, ensuring all assessments, diagnostic investigation and test results are up-to-date and in place to ensure a smooth admission process.
6. Liaise with the Paediatric Bed Manager and ward staff to ensure appropriate and timely discharge/follow-up arrangement in place for patients
7. Contribute to the development of appropriate patient group directive to facilitate/enhance the discharge process. Person SpecificationEducationEssential
NMC part 1 ( children) A Clinical expert either through formal qualification (degree or masters ) and /or extensive clinical experience pertinent to haematology
