  • Job Title / Assignment: Community OT (community rehab)

  • Department / Specialty: Intermediate Care: Community Therapy Directorate: Community

  • Working Pattern / Hours: Flexible but ideally full time Mon-Fri 8am-4pm/9am-5pm.

  • Description of Duties / Work to be Performed: Community rehab, long term condition management, complex moving and handling, equipment prescription, posture management, home and environment assessments, vocational and cognitive management, seating, fatigue and anxiety management Specialist

  • Skills / Training / Experience Required: NHS experience in community OT with older adults.

  • Car driver to work across North East Hampshire with business insurance.







OT25




  • Working hours - Full time 5 days per week including occasional weekends

  • Duties - Extensive leadership experience, extensive community experience.

  • �Recent NHS experience. Complex manual handling, rehab, rapid response, elderly care, neuro, orthopaedics.

  • �Must be a car driver.







Responsibilities




  • Conduct comprehensive assessments to determine patients' needs, abilities, and goals

  • Develop and implement individualized treatment plans to address patients' functional limitations

  • Utilize evidence-based therapeutic techniques to help patients regain and enhance their physical, cognitive, and psychosocial skills

  • Provide education and support to patients and their families, empowering them to participate actively in the rehabilitation process

  • Collaborate with healthcare professionals to ensure an integrated approach to patient care

  • Regularly evaluate patients' progress and adjust treatment plans as necessary

  • Maintain accurate and timely documentation of patient evaluations, progress, and outcomes
