Must be qualified RMN, social worker or OTRelevant registrationCommunity experienceCare coordination experienceEPJS experienceSouthwark/SLAM experienceRehab/placements experienceSkills – care coordination, documentation, risk assessment skills, care planning skills, communicating and team working skills with mdt and other mh teams/wards and agencies (incl. supported accommodation staff), lone working, able to travel around the borough, organised, flexible, engagement skills, physical health skills, meds management skills
