Position Overview:
We are seeking an experienced and compassionate Adult Senior Practitioner Social Worker to join our dynamic Neighborhood Team. In this role, you will take a lead role in providing expert social work support to adults facing complex challenges, including mental health issues, disabilities, and social isolation. You will work closely with individuals, families, and community partners to empower adults to overcome barriers and thrive in their communities.
Key Responsibilities:
Lead the delivery of social work services for adults within the Neighborhood Team, ensuring high-quality, person-centered support.
Conduct comprehensive assessments of adults' needs, strengths, and goals, and develop individualized support plans in collaboration with service users and their families.
Provide expert guidance and supervision to social work staff, supporting their professional development and growth.
