Mid Furness ICC's core team is a small multidisciplinary team that supports and promotes collaborative working between health, social care, other agencies and the community. In addition the team attends external health promotion events and we even organise our own too, occasionally you would be expected to support these. A permanent opportunity has arisen for 2 Care Coordinators to join the team. We are looking for a friendly and proactive team member to assess, plan, implement and evaluate evidenced based patient care, liaising with the multidisciplinary team within a community setting, supporting our residents to live independently. Based within the Ulverston Health Centre with travel across Ulverston, Dalton in Furness, Askam in Furness, Broughton in Furness and surrounding villages.Main duties of the jobThe ICC has an extensive network of local service providers which you will utilise to implement the anticipatory care plans. The roles will involve liaising closely with other providers from both statutory and voluntary agencies. We are looking for 2 Care Coordinator who are

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services. WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses. All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies,daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.Job descriptionJob responsibilitiesAs the Care Coordinators for Mid Furness Integrated Care Community (ICC) you will meet with people in their homes to unravel their health and social needs. You will coordinate their care, offer advice, and organise additional support. As the Care Coordinator you will work with families to support their relative and keep them independent. You will undertake fullholisticassessments ofhealth and social needs,whichonce documented willcreatean anticipatory care plan.Thisplan will be shared with other health and social care professionals to communicatethe personsinformed choices about how and where they want to besupported in the future.
