Adult's Learning Disabilities & Autism Advanced Practitioner

Somerset County Council are looking to expand their current team. The Adult's Learning Disabilities & Autism team is looking for a Advanced Practitioner to join a well-structured and organised team. This Adult's Learning Disabilities & Autism Advanced Practitioner role will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance.

Adult's Learning Disabilities & Autism Advanced Practitioner Responsibilities Include:

  • Working collaboratively across the Council, with partners and statutory and voluntary agencies, to ensure a joined-up approach.
  • Taking part in a range of case conferences, court attendance, meetings and statutory reviews, pro-actively taking the lead when a social care perspective is required.
  • Developing a high level of expertise in particular areas of practice, and leading on those areas
  • They will provide supervision and support for less experienced staff, support students and take on the most complex cases.
  • To provide Case Management, Health Action Planning and Person Centred Planning.
  • To participate in Continuing Health Care (CHC) Assessments where appropriate.
  • To assess, according to the agreed eligibility criteria and organise the equipment, adaptations and services required in a safe, timely and effective manner.
  • To develop an understanding of a variety of methods of overcoming and managing problems of daily living (using alternative techniques, arranging equipment, equipment trials, minor adaptations, home care services, re-ablement, direct payments) and be able to advise (and as relevant demonstrate to) clients, relatives and carers on their application, suitability and safe usage.
  • To participate in the Community Learning Disability Team's duty service.

Requirements / Experience preferred:

  • HCPC Registration
  • 1-2 years post qualifying experience within the UK Social Care industry
  • Enhanced DBS/CRB (we can obtain for you)
  • Eligibility to work in the UK.
