Job summaryAn exciting opportunity has arisen for the Clinical Lead for Westminster Joint Homelessness Team (JHT). We are looking for a nurse with robust clinical expertise who is also able to demonstrate progressive skills in leadership and service development and who has experience in, or looking to develop skills in, working with people with mental health difficulties who are street homeless.The JHT, based at 190 Vauxhall Bridge Road is looking for a passionate candidate who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities. CNWL is passionate about developing its own staff, driving them to go on to become more established practitioners. This is a great opportunity to take your next steps in CNWL.The JHT is a specialist team working with Westminster rough sleepers and works closely with social care colleagues within the team and with other partner agencies to ensure those who are street homeless receive assessment and treatment for their mental health needs. The team provides an integrated health and social care approach including outreach to those who are street homeless.The post is open to registered mental health nurses.Main duties of the jobThe successful applicant may have contact with patients or service users. To advise the JHT manager regarding the delivery of community mental health services and be involved in identifying the resources needed in the team.To ensure that the service works in co-ordination with other Community and Acute In-Patient services.To ensure effective liaison with other relevant agenciesTo maintain and develop administrative procedures regarding the recording of case records.To ensure that the service works in a co-productive way with service users, their families and stakeholders in the development of the team and local servicesTo facilitate the development of effective supporting structures for carers in line with CNWL and WCC guidelinesJob descriptionCLINICALSTANDARD:Foster and implement a multi-disciplinary team approach in all aspects of clinical and non-clinical performance.INDICATORS:Screen and accept referrals and undertake initial assessment in accordance with the Teams Operational Policy.Work as a leading member of the multidisciplinary team and work with other team members, to participate in the formulation, review, implementation and monitoring policies for the service.Provide immediate support to patients, relatives and carers.Follow up will use appropriate evidence based clinical skills.
