Job Description:
As an Intake and Assessment Social Worker, you will play a crucial role in the initial assessment and response to referrals concerning children and families in need of support or intervention. Key responsibilities include:
Conducting comprehensive assessments to evaluate the needs and risks faced by children and families.
Collaborating with multi-disciplinary teams to develop and implement appropriate intervention plans.
Providing support, guidance, and advocacy to children and families to promote positive outcomes.
Liaising with external agencies and stakeholders to coordinate services and resources.
Maintaining accurate records and documentation in accordance with statutory requirements and agency policies.
