Job Description:

Are you a compassionate and skilled social worker looking for a rewarding opportunity in healthcare? We are seeking experienced Hospital Social Workers to join our team in North Tyneside.
In this role, you will play a crucial part in providing support and assistance to patients and their families, ensuring they receive the care and resources they need for a smooth transition during their hospital stay and beyond.

Responsibilities:

  • Conducting comprehensive assessments of patients' social and emotional needs, including factors such as housing, finances, and support networks.
  • Collaborating with healthcare professionals, patients, and their families to develop and implement personalized discharge plans.
  • Providing advocacy and support to patients and their families, helping them navigate healthcare systems and access relevant services and resources.
  • Liaising with community agencies and service providers to coordinate ongoing support and follow-up care for patients post-discharge.
  • Maintaining accurate and up-to-date records of assessments, interventions, and discharge plans.

Qualifications:

  • Qualified Social Worker registered with Social Work England.
  • Experience working in a hospital or healthcare setting, preferably in a social work capacity.
  • Strong assessment and communication skills, with the ability to engage effectively with patients and their families.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Empathy, resilience, and a commitment to providing high-quality care and support to patients and their families.
