• Qualified Social Worker required to join a statutory Hospital Discharge Team
• Office base: Morden, London Borough of Merton
• To be considered, you must have a social work qualification and full SWE registration as well as over 3 years post qualified experience gained in a Hospital setting
• 3 month initial contract
• 35 hours per week
