Salary: �26 per hour
Are you a compassionate and dedicated social worker looking for a rewarding opportunity to make a difference in the lives of patients and their families? We are seeking a skilled Hospital Social Worker to join our dynamic team in North Tyneside.
Responsibilities:
- Conduct psychosocial assessments to evaluate the needs of patients and their families.
- Provide crisis intervention and emotional support to patients and families facing difficult medical situations.
- Collaborate with healthcare professionals to develop and implement discharge plans that ensure patients have access to appropriate support services upon leaving the hospital.
- Advocate for patients and families to ensure their voices are heard and their rights are respected within the healthcare system.
- Facilitate communication and coordination between patients, families, healthcare providers, and community resources to promote continuity of care.
- Assist patients and families in accessing financial assistance, housing, transportation, and other resources to address social determinants of health.
- Document interventions, assessments, and discharge plans in accordance with professional standards and regulatory requirements.
A strong Candidate will have:
- Previous experience working in a similar role within a healthcare or social care setting
- Knowledge of relevant legislation, policies, and procedures related to adult social care and safeguarding.
- Strong assessment and decision-making abilities, with a person-centred approach to care.
- Excellent communication and interpersonal skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.
Requirements:
- Registered with Social work England
- A Degree in Social work or Diploma in SW, CQSW, CSS
- Right to Work
- Enhanced DBS check, dated within the last 12 months
