Health and Wellbeing Practitioner Inclusion Lead (Heathrow IRC) Location: West Drayton, Middlesex Salary: £25,154 per annum Are you looking for experience in forensic psychosocial interventions? Looking for a dynamic, creative and multicultural working environment?

What you will be doing Working within an integrated healthcare framework the Forward Trust provide the Substance Misuse Recovery Services at Heathrow IRC. The role is based on site at Heathrow Immigration Removal Centre. As Health and Wellbeing Practitioner you will work as part of a team in supporting detainees holistically in relation to substance misuse, low-level mental health and overall health and wellbeing issues. Day to day you will be responsible for: Attending daily referral meetings & team huddles as required Attendance and contribution to multi-disciplinary reviews Complete assessments of need, risk management and recovery planning Caseload management and delivery of both group and 1:1 interventions Promotion and signposting of services Discharge and release planning to provide effective continuity of care Record keeping and case note entry in line with data protection and GDPR requirements Contribution to overall service improvement and organisational objectives Undertake relevant training and development Provide transitional support from the detention centre to the community to prepare service users for release or deportation. Report writing and engagement summaries for official purposes Your role as inclusion lead will involve additional responsibility for: Taking initiative to address any gaps in service to ensure inclusive provision Liaising with management and the team to ensure effective service delivery to all service users groups Take forward any developments for new resources or identified needs Plan and deliver service user forums to gather feedback to inform service improvement planning Act as a single point of contact for any issues or queries raised in relation to inclusion and diversity Contribute to and promote the organisational ED&I strategy Coordinate events and activities monthly that promote diversity and multi-cultural awareness across our team and the wider community within the centre What we are looking for Knowledge of the issues facing substance misusers and low-level mental health issues Demonstratable experience of carrying out risk assessments, comprehensive assessments and the design and implementation of SMART care plans. Significant experience of carrying out psychosocial interventions including using motivational interviewing techniques in both 1:1 and therapeutic group settings. A thorough knowledge of and commitment to the 12-step process of recovery from addiction. Experience of providing counselling / CBT services to prisoners, or those with Health and Welling, or drug and / or alocohol problems Have lived in the UK consecutively for the last 3 years
