Balfor Recruitment are Recruiting for a Health and Social Care officer based in Nottingham.the ooprtunity for a Health and social care officer is fulltime (37 hours per week) and the hours will be from 9am - 5pm Monday to Friday.
This role is office based and you will be assisting in the delivery of an efficient and effective referral service for adults with complex needs, often dealing with difficult and unpredictable situations.
You will need to have a good understanding of the variety of services that are available to support these vulnerable adults.
As a Health and Social Care Officer working within Nottingham you will be taking referrals for adults across the city including some of the most vulnerable citizens. You will triage, evaluate, and navigate citizens through to the right services depending on their needs.
As a Health care officer, you will be required to:
1. To coordinate telephone calls, enquiries and referrals from citizens, vulnerable adults, families and carers and to manage these in an effective, timely and customer focused way.
2. To deliver an agreed level of customer service that reflects the vision of the organisation
3. To manage specialist or escalated calls from Health and Social Care Support Co-ordinators, Health and Social Care Support Advisors or other colleagues.
4. To have an understanding and knowledge of the variety of relevant services and their referral criteria available within social care and health to enable appropriate advice and guidance is given and to ensure this information is kept up to date.
5. Liaising with and working with relevant clinicians, practitioners and health and social care provider services to coordinate appropriate care packages and ongoing care needs, this will include triaging and signposting citizens to other services, tracking and monitoring pick up and handover of clients to the crisis response team.
6. To make judgements regarding referrals and requests in relation to complex situations, safeguarding concerns and risk management by triaging and coordinating them to make sure that clients are seen by the most appropriate team / service at the right time to meet their needs.
7. To be able to handle telephone calls from distressed referrers, citizens, carers who are experiencing potentially complex problems and act appropriately in a supportive, empathetic and courteous manner. To be able to alert team members, supervisors of any relevant issues arising.

Requirements
* Foundation degree or NVQ3/equivalent and/or demonstrable and extensive experience in an administrative/business/marketing/customer care or social care environment.
* strong organisational, administrative and interpersonal skills.
* Enhanced DBS Registered onto the update service.
