An opportunity has arisen for a Band 6 Care Coordinator working within the Early Intervention in Psychosis Team based at Parklands House in Basingstoke. The role will involve working across the Mid and North of Hampshire area. The work is varied and interesting and there is a strong team approach ensuring good support for the successful applicant. The role requires lone working, use of initiative and a commitment to team working and service user empowerment. There will be an opportunity to develop your skills working in this specialist team and to maximize your knowledge of working with psychosis. The post holder will be from a mental health nursing background. You will need to be able to work flexibly and to demonstrate knowledge of the CPA process, risk assessment, and the recovery model. This post involves weekend and bank holiday working on a rota basis as per service user need.Main duties of the jobTo provide care co ordination for people in the first three years of a psychotic illness. To provide specialist evidence based clinical interventions to young people and their families in conjunction with early intervention core team specialists and Support Time and Recovery workers and community partners. *Minimum in depth post qualification experience *Experience of working with people with psychosis and knowledge of psychotic illness and symptomology *Experience supporting clients through crisis points in mental health *Knowledge of delivering psychosocial interventions for psychosis

Additional CriteriaEssential
*Sound understanding of core components of Early Intervention in Psychosis *Excellent verbal and written communication skills, including report writing and presentations *IT skills *Ability to assess, plan, implement and evaluate a comprehensive care package for the identified client group, using a wide range of resources
