Do you enjoy a fast-paced environment?We have two exciting opportunities available to join our Emergency Assessment Unit in the Emergency Care Centre as a Discharge Coordinator. These dedicated posts are key to facilitating safe, timely discharges for our patients, a vital part of the patient journey.Main duties of the jobWorking collaboratively with the ward nursing team, the successful candidate will be responsible for coordinating the discharge process on the unit, liaising with patients and families, as well as multiple internal and external teams (for example, Patient Flow team, Discharge Liaison Nurses, external care providers, social services etc.) to ensure that patients have everything required for a safe discharge to the required location.Working for our organisationBased in the North East of England we provide a range of hospital and community health services from our leading facilities including the Queen Elizabeth Hospital, Blaydon urgent care Centre and Bensham Hospital, all within Gateshead.Detailed job description and main responsibilitiesThe role requires an individual with exceptional communication skills, able to effectively prioritise and manage their own workload, and maintain the delivery of a high-quality service throughout times of pressure. The successful candidate will be able to proactively identify concerns/barriers to discharge and provide solutions where possible through building relationships with all the identified teams.
