An exciting opportunity has emerged to introduce a progressive role to support adult diabetes services in the acute sector.We are looking for two Band 7 Diabetes Specialist Practioners (Diabetes Specialist Nurse, Dietitian, Pharmacist or Podiatist) to join our team at Imperial College Healthcare NHS Trust (ICHT).Band 7 role requires you to act as an autonomous expert practitioner in forming decisions and complex management of Type 1 & Type 2 diabetes across inpatient and outpatient settings. We are seeking an expericned DSP with an interest in inpatient to lead this aspect of the diabetes service at Charing Cross Hospital. Both posts will require onsite working Monday to Friday with occassional cross site working.Working as part of a multidisciplinary team with consultants, podiatrists, dieticians and other professions, the diabetes nursing team and Diabetes Specialist Practitioners perform a crucial role in making a difference to patients with diabetes across inpatient wards and Northwest London service provider. The changing landscape of diabetes care provision is exciting, dynamic and lends the way for exploring more creative ways of working.Main duties of the jobProvide specialist diabetes practitioner advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision in collaboration with the multi-disciplinary team in line with local and national targets and in accordance to NHS standards and guidelines.Assess, plan, implement and evaluate programmes of care for adults with diabetes, through the development of individual care plans in partnership with the patient, their family and relevant others - including structured education.Deliver on a set of roles and responsibilities held within a defined portfolio of service responsibilities and maintain high standards of care in diabetes, demonstrable by benchmarking and audit, as stipulated in local and national guidelines.About usAt Imperial College Healthcare you can achieve extraordinary things with extraordinary people, working with leading clinicians pushing boundaries in patient care.
