Job Summary



Join our dynamic team at CSH Surrey as a full-time, permanent Band 6 Diabetes Specialist Nurse and make a significant impact on the lives of those living with diabetes.

Main Duties of the Job



As a Diabetes Specialist Nurse, you will:




  • Conduct advanced clinical assessments and create personalized management plans for your patients.

  • Interpret clinical signs, laboratory results, and other functional measures to initiate appropriate treatments and interventions.

  • Make autonomous, safe, and effective decisions to enhance patient health outcomes.

  • Communicate complex and sensitive information to patients, their families, and carers with empathy and clarity.

  • Organize and prioritize your workload, with flexibility to meet service needs and uphold quality standards.

  • Lead and mentor other healthcare professionals, enhancing their skills and contributing to community and primary care development.

  • Drive the adoption of evidence-based clinical practices and develop specialist pathways within the service.



Experience



Essential Criteria




  • A minimum of 5 years experience as a Registered Nurse.

  • Evidence of working within a multidisciplinary team.

  • Experience in managing patients with diabetes, including both type 1 and type 2.



Desirable Criteria




  • Research or clinical audit experience.

  • Community healthcare experience.

  • The ability to clinically assess, deliver, and evaluate high-quality care.

  • You must be able to work autonomously, reflect on your strengths and development needs, and work under pressure while balancing multiple priorities and meeting deadlines.

  • Demonstrateable evidence-based practice in line with clinical governance principles.

  • A willingness to develop new skills appropriate to the role is required.

  • Confident in using information technology

  • Valid UK driver's license with access to private transport for business purposes.

  • Excellent communication and interpersonal skills.



Qualifications



Essential Criteria




  • NMC registration as a (RGN) Registered General Nurse.

  • A degree in Nursing or an equivalent level of experience.

  • Evidence of certified (CPD) continual professional development within diabetes.



Desirable Criteria




  • A postgraduate certificate in diabetes.

  • Completion of a mentorship course.
