As part of this strategy, we are proactively taking steps to ensure that we recruit and retain the highest quality of talent to provide CARE and we would welcome applications from under-represented groups to ensure our Trust is an inclusive and diverse employer.Detailed job description and main responsibilitiesYou will accept referrals from GPs, Consultants and other Healthcare Professionals for the highly specialist management of diabetes.Provide highly specialist nurse led clinics in which care is planned, initiated and assessed by the DSN, some of which is the sole specialty of the DSN and is not provided by healthcare providers in the community, island wide e.g. patients with complex diabetes needs.Provide specialist education and training for patients and their carers and relevant health care professionals. To act as an expert resource and to provide highly specialised advice about diagnosis and management of diabetes to other health professionals in primary and secondary care including general practitioners, nursing staff (practice nurses, district nurses & midwives) and other members of the multi-professional team such as podiatrists and dieticians.You will support leadership to enhance the development of the service.Also provide the highest level of expert clinical practice underpinned by evidenced-based research.Undertakes research, clinical audit and evaluation of education in the area of diabetes.Person specificationQualifications and role specific knowledge*Registered Adult Nurse on the NMC register.Completed a University Diabetes Course to level 3.Worked as a Diabetes Specialist Nurse in secondary or primary care.Non-medical prescriber or be willing to undertake training.Physical assessment and history taking course or equivalent experience or be willing to undertake.Physical SkillsHave proficient computer skills and able to use the trust IT systems.Be able to collect data & collate information.Be willing to undertake training in equipment used within the Diabetes Service including blood glucose meters and insulin delivery devices.Insight into job role.Enthusiastic and self-motivated.Mature approachAble to work on own initiative and take responsibility when appropriate.AbilitiesDemonstrate the ability to organise, prioritise and manage own workload.Good team worker To be able to work by oneself and as part of a team.Manage own caseload and clinics.Communication SkillsPolite/friendly manner with excellent communication skills.Be discreet and able to always maintain confidentiality.To convey and discuss sensitive information and diagnoses at differing levels.Communicate effectively across the Islands healthcare system.QualificationsRegistered Healthcare ProfessionalEmployer certification / accreditation badgesYou must have appropriate UK professional registration.This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
