We are looking for an exceptional Band 6 District Nurse Deputy Team Leader to join us in our Central London community nursing service in West London which covers Kensington & Chelsea and North West Westminster.If you are a passionate, motivated team player and want to excel in delivering exceptional healthcare to a diverse patient group alongside a range of community experts and team leads, we want to hear from you.The District Nurse (DN) is a leader within the nursing team. The post holder will be expected to undertake the holistic assessment of patients using a person centered approach, and co-ordinate care packages across both health and social care as appropriate to avoid inappropriate admission to hospital or long term care environments, in addition to facilitate the timely discharge of patients from secondary care whilst taking active part in innovative projects that enhance patient centered service delivery and development.
