The RFH is a dedicated heart attack centre covering a population 1.5 million people across North central London and is one of the busiest of London’s seven HACs. In newly renovated Cath labs with the latest Siemens ICONO system, integrated intracoronary physiology (FFR, Coroventis) and imaging (OCT, IVUS) is routinely applied and we undertake the full range of complex and high risk PCI with access to calcium modification (rotational atherectomy, Shockwave), CTO and LMS PCI. We perform more than 1000 PCI per annum. The RFH is a large teaching hospital and is a campus of UCL medical school with an active research programme. The cardiology department includes onsite advanced cardiac imaging including CMR, 3D echo, TOE, CTCA and SPECT. There is a full EP service including complex devices and AF ablation. The RFH is a national referral centre for amyloid and pulmonary hypertension and we have been at the forefront of developing thrombectomy services for acute PE, although not part of this post. The successful candidate will participate fully in the PPCI oncall rotation (1 in 7.5) and also the cardiology consultant of the week (3 weeks per year across sites).There is no general medicine component to this job. A higher degree is desirable and the successful candidate will be encouraged to participate in research and teaching.Main duties of the jobThe Royal Free represents the largest stand-alone PCI in London with off-site surgical cover provided by surgeons at Barts with whom there are strong links including a regular JCC meeting. There is a full invasive and interventional programme including diagnostic angiography, PCI, permanent pacemaker systems, complex device and EP programme. In newly renovated Cath labs with the latest Siemens ICONO system, integrated adjunctive technology including intra-coronary physiological measurements (FFR, Coroventis etc), and imaging (IVUS, OCT) are routinely applied. Calcium modification with rotational atherectomy and intravascular lithoptripsy (Shockwave) is available and we also have a programme of complex PCI managing LMS, CTO and bifurcations with a dedicated PCI MDT. Elective and inter-hospital transfer patients are treated as ‘day cases’ via a dedicated day interventional unit. We are also developing a virtual pathway for low risk NSTEMI cases (ALTAS). The department is one of the busiest of London’s dedicated Heart Attack centres providing 24/7 on-call emergency services for primary PCI and the appointed candidate would participate in a 1:7.5 PPCI oncall rota. We have been at the forefront of developing thrombectomy services for acute pulmonary embolism and further training in this field would be available for interested candidates although will not form part of the core job plan.Working for our organisationThe Royal Free is one of the designated London Centres for the National Pulmonary Hypertension service particularly specialising in the treatment of patients suffering from scleroderma-associated pulmonary hypertension. There are strong links with the academic department of rheumatology. In addition, a unique cardiac clinic has been developed for patients with Carcinoid, in conjunction with Royal Free national centre for the management of neuroendocrine tumours (Prof. Caplin). A joint appointment exists between cardiology and the National Amyloid Centre. There are strong clinical and academic links with clinical biochemistry as the Royal Free is a designated centre for the management of familial hyperlipidaemia (Dr Nair).Detailed job description and main responsibilitiesTo provide overall care for patients referred to and cared for by the cardiology services including inpatient, outpatient and community services at Barnet and the Royal Free, to support the development of a robust MDT oversight regime, to support the nursing and administration teams in delivering safe care and to support research in this field.It is specifically anticipated that with their colleagues the post holder will provide the following services:Supporting activity through catheter lab located within Royal Free HospitalParticipate in full slot of PPCI rota (1:7.5 with prospective cover)Contribute to the general cardiology at the Royal Free site as ‘Consultant of The Week’ providing daily CCU and cardiology routine and post-take ward rounds and providing a consult service for the acute medical take.
