Eliza Assessment Service is seeking to employ an enthusiastic and motivated Comprehensive Assessor to complement the existing multi-disciplinary team. Practicing within the framework of My Aged Care and within Peninsula Health organisational requirements, the Comprehensive Assessor will provide a comprehensive and independent assessment of the aged client (and some younger adults when appropriate), and with the client develop care plans that promote the restoration of function and/or maximise independence. The Comprehensive Assessor will also review support plans and facilitate referrals or provide service coordination for older people to engage a range of community or hospital-based services in order to meet their specific needs.The Mt.
