THE ROLE
To be responsible for providing comprehensive mental health specialist care within the organisation. Youwill be expected to develop, plan, coordinate and deliver the service whilst working as part of theorganisation’s multidisciplinary team.The Mental Health Practitioner will work as an autonomous practitioner and be responsible for theprovision of assessments, interventions and education. They will have responsibility for managing acaseload of patients with a variety of mental health conditions and will be expected to collaborate withother healthcare professionals both internal to, and external from, the organisation.To support the management team in the reviewing of clinical policy and procedure.

JOB RESPONSIBILITIES
Primary Key Responsibilities:The following are the core responsibilities of the Mental Health Practitioner. There may, on occasion, be a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.• To support practice staff in the management of mental health conditions• To evaluate and prioritise patients’ symptoms and provide general advice and reassurance in line with up-to-date clinical research guidelines• To provide appropriate signposting to the relevant clinician or external organisation whilst ensuring that the patient receives appropriate ongoing care• To promote high standards by providing the assessment, treatment and management plan as per current NICE guidance that is specific to primary care• To support primary care initiatives which improve the physical health outcomes of those with a long-term mental health condition and the mental health outcomes of those with a long-term physical health condition• Responsible for the management of the mental health, learning disability and dementia registers, providing annual reviews in all aforementioned areas• Working with young people and children and ensure they have access to support• To maintain clear, concise and accurate documentation within the systems provided• To ensure that all necessary arrangements are completed promptly and in a safe manner• To maintain effective communication with multidisciplinary teams to the benefit of the patient• To maintain the confidentiality of matters relating to patients and patient care in accordance with the NMC/HCPC guidelines• To communicate complex and sensitive information concerning a patient’s medical condition effectively to patients/carers, recognising that sometimes there are barriers to understanding• To foster good public relations by ensuring courtesy and discretion when dealing with patients and their relatives• To support and advise patients on the promotion of health, prevention of ill-health and the self management of their health within their limitations• To recognise situations that may be detrimental to the health and wellbeing of the individual and act on the findings• To ensure that care given is supported by the best available evidence and local policies and procedures• To deliver opportunistic health promotion where appropriate• To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately• To uphold the confidentiality of all records/information held by DMC Healthcare. This duty lasts indefinitely and will continue after you leave your employment with this organisation. All information which identifies individuals in whatever form should be managed locally with the Confidentiality Policy and in general in accordance with the Data Protection Act 2018• To maintain professional competency whilst remaining current and in date for NMC revalidation (as required)
Secondary Key Responsibilities:In addition to the primary responsibilities, the Mental Health Practitioner may be requested to:• Support the nursing department• Support junior members of the nursing team, providing guidance when necessary• Participate in local initiatives to enhance service delivery and patient care• Support and participate in shared learning within the organisation• Continually review clinical practices, responding to national policies and initiatives where appropriate• Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA)• Support other clinical governance activities within the organisation
PERSON SPECIFICATION
Qualifications Essential
• Registered Mental Health Practitioner (RNM) with a current NMC registration or Occupational
Therapist with the Health and Care Professional Council (HCPC) or other regulatory body.
Membership of relevant professional body
Experience Essential
• Minimum of 2 years’ experience post qualification in a clinical setting
• Ability to administer medications including IMI depot medication
Experience Desirable
• Experience of working in both a primary care and community setting
• Liaising with a variety of stakeholders
Clinical Knowledge and Skills Essential
• Ability to work autonomously and use own initiative
• Advanced understanding of mental health problems and treatment options/models that can be
used to assist delivery
• Ability to confidently articulate clinical opinions in formal and informal meetings or exchanges
• Ability to record accurate clinical notes within appropriate timescales
• Ability to analyse and resolve professional and ethical issues
• Understanding of relevant professional codes of ethics and conduct, professional standards and
core skills and how these are applied to clinical practice
• Knowledge of relevant legislation, joint working practice and government initiatives and policies
relating to mental health
• Ability to work within own scope of practice and understand when to refer to GPs
• Understand the importance of evidence-based practice
• Ability to communicate complex and sensitive information effectively with people at all levels by
telephone, email and face to face
• Ability to identify risk and assess/manage risk when working with individuals
• Ability to take part in reflective practice and clinical supervision activities
• Demonstrate personal accountability, emotional resilience and work well under pressure
