Partnership working is our core to our values and the Community Matron team is paving the way towards integration, shared care and breaking down organisational barriersMain duties of the jobWe are looking for a highly skilled Nurse with (or prepared to develop) advanced assessment and prescribing skills to join our well established team. You will work with our multidisciplinary Integrated Domiciliary Service (IDS) which utilises technology via virtual MDTs to improve communication and response time and enables clients to remain in their own homes. You will work closely with Community Services, Primary Care and the Voluntary Sector to provide case management for patients with complex needs. You will be responsible for setting high standards of practice and be a flexible and adaptable team player.
