The position involves providing leadership support to the team on a day to day basis and to support the Integrated Community Lead as required.Romsey ICT is part of Southern Health NHS Foundation Trust and work as an Integrated Community Care Team including, Community Nursing, Occupational Therapy and Physiotherapy, Frailty Team. We deliver these services to our community so you will feel a real sense of achievement and that your work makes a difference to local people's lives.Main duties of the jobProvide professional nursing leadership within the team and support the Community Matron in managing the team and ensuring safe and effective staffing levels and provision of resources to ensure continuous service delivery and enhancing nursing practice.Participate in the holistic assessment, implementation and evaluation of programmes of care for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needs for example the physically disabled. Other settings include residential homes, nursing homes, clinic's community centres and community hospitals.To provide complex case management using extended skills where appropriately trained to avoid hospital admission and manage complex nursing needs in the community setting.Maintain upheld and follow the standards within the NMC code of conduct at all times ensure safe and effective delivery of patient care.Review effectiveness of nursing care provided ensuring that patient safety is maintained and care delivery meets the standards required by the Trust and Care Quality Commission.Advise on the promotion of health and prevention of illness and provide information to individuals and groups to prevent disease, where possible. A minimum of four years post registration experience Experience of caseload management including responsibility for complex care packages for vulnerable people Experience, underpinned by knowledge of working with and understanding the complex needs of patients in a primary care/community setting Involved in the implementation and management of change Evidence of innovative practice Participates in regular clinical supervision Experience of working with long-term conditions and frailty
