The Seaton Hospice at Home team is managed by the Royal Devon University Healthcare NHS Foundation Trust but but is funded by the Seaton League of Friends. The team provides a specialist twenty-four hour holistic and patient-centred palliative care service for all patients who are registered with a Seaton GP and diagnosed a palliative, regardless of disease or complexity, guided by the individual needs and wishes of the patient and their loved ones. This service provides continuity of care throughout the patient's journey, enabling people to die in their preferred place, which is often their home. Working Pattern: 30 hours per week. The team work three different shift patterns: 08:00-16:00 10:00-18:00 and 12:00-20:00. After 20:00 hours a Registered Nurse is on call to respond to patients who need advice or a visit overnight, thus providing specialist end of life care and support 24/7, avoiding unnecessary and unwanted hospital admissions.
