Clinical Nurse Practitioner

We are looking for a vibrant and dynamic person to join our award winning, pioneering and integrated team in this exciting role. The post holder will develop, implement and evaluate a seamless specialist service, ensuring that patients receive the highest standard of clinical care in their usual place of residence. This includes a 2 hour response for patients referred to the service as part of our Urgent Care Response pathways which responds to patients who have both health, therapy and social care needs. The successful candidate will demonstrate a high level of expertise and skill in their work providing advice, education and support to health care professionals and patients. They will actively undertake clinical audits and team management, working to set and maintain high standards both clinically and non-clinically. This role will predominantly be based in the community but will rotate across other Band 7 roles within the team, which are Duty clinician which requires the coordination of the team's clinical caseload, Triage Nurse which involves speaking with and managing referrals made to the team and the In-Reach Nurse role. This role located within Kings College and Guys and St Thomas' Hospitals and is where the service proactively works with acute partners to identify and triage patients living in Lambeth and Southwark who are suitable for expedited discharge from hospital supported by the @Home service. This position includes a rotation through all Band 7 roles within @home service, in addition to direct patient contact you will be expected to complete the UPA triage role, In-Reach role (based in GSTT or KCH) and duty clinician role. You will be expected to act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines. To be pivotal in encouraging and managing appropriate referrals, attending ward rounds, MDT and other meetings. Ensuring that patients receive high quality clinical care and a good patient experience, having regard for their social circumstances, customs, religious beliefs and doctrines. To work autonomously, managing patient assessments within the specialty whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care To act in accordance with the extended role of the nurse and be competent in extended clinical skills including nurse prescribing, AED, Phlebotomy, Cannulation, IV Drugs, male and suprapubic catheterisation, ECG and Fine Bore Tube. Physical manual handling of patient, patient movement and the care of mechanical aides. Act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines. Participate in the promotion of patient-centred care, and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and transfer of care Support and enable patients and carers to make informed decisions relating to their treatment and management. Promote high quality clinical care, and assist in the development of the specialist service, through the use of evidence based practice, risk assessment and clinical standards. Provide professional and clinical leadership and expertise in the speciality and to act as a resource for education and clinical expertise in the hospital and community settings. Demonstrate appropriate expert practice and highly specialist advice in the assessment, planning, implementation and evaluation of patient care. This includes history taking, physical examination and the requesting of agreed diagnostic procedures/clinical investigations. Responsible for the correct administration of prescribed medication. Promote and deliver high standards of care at all times, which utilises research-based evidence, and is audited and improved accordingly. Act as a role model, demonstrating high standards of care and providing clinical leadership in the specialist areas. Establish, develop and maintain effective working relationship between clinicians, practitioners and managers within the specialty, across the Trust and KHP (Kings Health Partners). Actively participate in local, national and international networking to maintain and develop expert level of knowledge in the specialty. Provide a seamless, high quality service from referral through to assessment, diagnosis, treatment and review. Develop and evaluate assessment tools that will ensure an appropriate level of nursing intervention so that patients who present with the most complex needs receive the most appropriate specialist care. Work in conjunction with the clinical consultant to lead a patient focused service. Act as patient advocate when appropriate, respecting patient confidentiality and privacy with respect for diverse cultural backgrounds and requirements. To be designated to take charge of an area within the @Home service in the absence of the person holding continuing responsibility.
