Are you looking for a step up into a Clinical Lead / Head of Care role you will provide leadership, direction, and guidance to the care managers across the group and will be responsible for managing THREE residencies.
I am recruiting for a Clinical Lead / Head of Care to join an established, growing organisation with an excellent reputation, that provides residential care for Adults just outside of the Mold area.

Clinical Lead / Head of Care, you will be responsible for:

  • Provide leadership, direction, and guidance to the care managers across the group
  • Mentor and coach care managers - to include support through effective induction as required
  • Provide support, guidance, and assistance to the staff team
  • Troubleshooting daily as necessary
  • Ensure the home is suitably staffed
  • Support the effective resolution of team conflicts
  • Undertake On-Call as required to cover out-of-hours, attending the home as necessary.
  • Undertake care responsibilities as required
  • To meet with care managers on a weekly basis
  • Be familiar with the care and support needs of all individuals within the service;
  • Developing and updating care plans within the format provided.
  • Monitor and review service user needs and risks as appropriate.
  • Undertake and contribute, as required in the multi-disciplinary assessment of service user needs and the subsequent planning of service
  • Support and undertake as required initial assessments/pre-admission/pre-discharge assessments for new service users/service users that have been in hospital, preparing care and support plans and risk assessments as needed
  • Ensure good, stable occupancy levels in all services
  • Maintaining clear, accurate medication records
  • Medication management in accordance with CIW Regulations and company policies and procedures.
  • Interacting with professionals and relatives
  • Participate in CIW inspections and assist with compliance issues and actions
  • Ensure compliance with regulatory and legal requirements, safeguarding policies, deprivation of liberty safeguards, health and safety legislation, and best practices in social care.
  • Be responsible for the investigation and reporting of all incidents and accidents (including safeguarding), ensuring timely action is taken and documented and full 360 communication as required.
  • Participate in the induction and development processes of existing and new employees as appropriate, including observations, supervisions, and appraisals, you are responsible for all investigatory meetings passing on to the operational manager for disciplinary action where appropriate,
  • Attend regular review meetings, annual appraisals, team meetings, training courses, and follow-up refresher courses, as required.

Requirements:

  • Level 5 Diploma in Leadership and Management for Adult Care
  • Experience as either a Clinical Lead, Care Manager, Service Manager, or RM in adult residential Services
  • Proven track record of good or outstanding QCQ judgements
